Overview
Supports residents to maintain tenancy and prevent homelessness through case management.

Tasks
-Identify tenancy risks and work proactively with Housing Officers to prevent tenancy breakdown -Work alongside mental health specialists, NDIS providers, and community agencies -Be a trusted advisor to internal teams, sector partners, and support networks -Champion best practices, lead service improvements, and contribute to program evaluation -Deliver leading tenancy sustainment solutions with a trauma-informed, recovery-focused approach -Support up to 25 residents through person-centred support plans and collaborative engagement

What You Bring
-Exceptional communication, planning, and stakeholder engagement skills -Will require travel to the Greater South West Region -Ability to manage conflict, navigate challenging behaviours, and build trusted relationships -Relevant qualifications in community services, social work, or mental health -Knowledge in areas such as mental health, AOD, trauma, disability, ageing, or cultural safety -Proven experience in case management or tenancy support for people with complex needs
