The Role

Overview

Manages services and case plans for ~34 individuals with intellectual disabilities in CT.

Tasks

-Coordinates integration of day program, residential, medical and other services provided to each individual and ensures service delivery; -Maintains regular contact and provides supportive guidance to assigned individuals and their families; -Oversees and supports continuity of care and waiver benefits; -Gives information to individuals and/or call-in’s, guardians and families regarding their legal rights, departmental policies and procedures, services provided and encourages participation in service planning process; -Schedules program reviews and monitors implementation of specific program recommendations; -Develops social service evaluations and service recommendations; -Performs activities related to individual services plans for conformity with federal and/or agency standards; -Coordinates and monitors the delivery of waivered services to ensure the federal requirements for reimbursement are met and maintained and monitors completion of appropriate documentation; -May review individual service plans for conformity to federal regulations; -Stay connected! Log in daily to your JobAps Personal Status Board to track your status and check email (including spam/junk folders) for updates and tasks. -Ensures that legal and financial documents are completed in a timely manner; -Provides information and support to individuals and families in obtaining and maintaining social services benefits to ensure continued waiver eligibility; -collect and analyze large amounts of information; -Monitors development and ensures maintenance of individual files including required documentation; -Convenes, chairs and facilitates interdisciplinary/planning support team meetings to develop, review and/or modify individual service plans; -Provides guidance and support to individual and/or family members in locating appropriate providers in the community; -May coordinate the development and assist with the implementation of Plans of Correction; -Acts as liaison and provides technical assistance to service providers and monitors compliance with departmental and waiver policies and procedures; -Informs appropriate supervisor and regional administrative staff when services are not or cannot be provided; -Assists with identifying the need for evaluations, scheduling of evaluations and monitors completion; -Assists with responses to ICF surveys, inspections and IPR/UR as required;

What You Bring

-Considerable ability to understand and translate clinical findings and recommendations into program activities and develop realistic program objectives; -Ability to -Bilingual in Spanish -Experience working with the concepts of Person-Centered Planning, as well as translating clinical assessment tools and programmatic findings into realistic and measurable objectives. -Experience working within family structures to establish supports within the family home. -Experience with explaining complex service systems to peers, families or people with disabilities from various socio-economic and cultural backgrounds. -Considerable skill in facilitating positive group process; -Experience leading meetings and utilizing conflict resolution skills. -Incumbents in this class may be required to possess and retain a valid Motor Vehicle Operator’s license. -First (1st) shift -Experience in providing critical decisions regarding individual’s health and safety. -Incumbents in this class may be required to travel. -Referral Questions: This posting may require completion of additional Referral Questions (RQs), which must be completed by the questionnaire’s expiration date. If requested, RQs can be accessed via an email sent to you after the job close date or by visiting your JobAps Personal Status Board (Certification Questionnaires section). -utilize computer software. -Experience organizing and prioritizing a constantly changing workload. -Experience collecting, analyzing and disseminating large amounts of information/data. -interpersonal skills; -Experience with Abuse and Neglect reporting and notification. -A Master’s degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines and one (1) year of professional experience involving responsibility for developing, implementing and evaluating individualized programs for individuals with an intellectual disability in the areas of behavior, education and rehabilitation. -Considerable knowledge of services available to individuals with an intellectual disability; -Experience with working with Microsoft Office Suite, including Excel, Word, Outlook and Teams. -Incumbents in this class must be eligible for certification as a Qualified Intellectual Disabilities Professional as required by federal regulations. -Experience using a quality monitoring system to assess the effectiveness of supports and assure desired results are being achieved for individuals supported; and if not, developing a plan to address unmet needs. -Experience and knowledge of state and community services for children. -A Bachelor’s degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines and two (2) years of professional experience involving responsibility for developing, implementing and evaluating individualized programs for individuals with an intellectual disability in the areas of behavior, education and rehabilitation. -Experience identifying and accessing state, federal, and/or community resources and benefits available to individuals with intellectual disabilities. -Knowledge of -oral and written communication skills; -Experience and knowledge of the Department of Children and Families system, Carelon Autism supports and the Behavioral Services Program. -Experience with self- determination, self-directed supports and self- advocacy for people with intellectual and developmental disabilities. -Experience with problem solving and developing creative solutions while working independently. -Experience with organizing and concisely providing information to others using verbal and written communication skills. -Educational Credits: List your earned credits and degrees from accredited institutions accurately on your application.
