Considers the needs of the participant and requirements of the PACE Center. Communicates these changes to Transportation Manager.
Assists in scheduling vehicle maintenance and equipment to operate within manufactures recommendations.
Schedules Transportation for participants to and from the Day Center, medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care. Also Schedules Transportation for last minute emergencies that arise.
Functions as a member of the Interdisciplinary Team when appropriate.
Communicates with Transportation Manager daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests
Develops schedules to limit employee overtime hours as much as possible.

Familiar with Pace Care Scheduling Program to be able to gather information needed for timely and efficient individual transportation and home care schedules.
Must pass a background check and drug screen.
Skilled in establishing and maintaining effective working relationships with participants, caregivers, drivers and co-workers.
High school diploma or equivalent required;
Have a minimum of one years’ experience in working with the frail elderly.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Valid Driver’s License in state of hire.
