The Role

Overview

Provide psychosocial assessments and support to nursing/rehab residents.

Tasks

-Work directly with residents in addressing psychosocial and leisure needs -Back-up and coordinate admission process, room change notifications, hospital referrals & contacts, tours, etc. -Develop a social assessment for each resident and plan of care based on individual history and specific needs -Provide referral and community resource information to residents/families -Coordinate the discharge planning process -Coordinate the Medicaid application process -Advocate for and uphold Residents' Rights, including documentation and follow-up on resident and/or family complaints or grievances

What You Bring

-Ability to work independently, proactively, and with teams as required -Working knowledge of the health care system and levels of care -2+ years of experience working with older adults -Excellent communication, interpersonal and organizational skills -Database, spreadsheet, and Microsoft Office experience -Ability to work nights and weekends as needed -Bachelor's degree with a concentration in a Human Service related field
