The Role

Overview

LPN leads home care team, manages client intake, supervision, and coordination.

Tasks

-Effectively communicate with the RN Assessment Nurse to ensure appropriate client assignments occur -Directly communicate with the Administrative Coordinator regarding system problems, repairs, and troubleshooting as needed -Supervise a team of In-Home Aides/Companions -Ensure any observed or suspected abuse, neglect, or exploitation is reported to proper authorities as mandated as a health professional -Process referrals for Home Care within one business day -Establish and maintain communication with clients and their family members/POA's regarding their care -Support the Home Care program with initial client intake -Participate in ongoing education and training offered by Givens and/or outside resources as required -Develop and maintain strong relationships with referral sources

What You Bring

-Must be currently licensed in the State of North Carolina as an LPN -Active CPR (cardiopulmonary resuscitation) certificate or willingness to obtain within six months of joining Givens -Strong computer and interpersonal skills -Previous experience leading a team, and experience supporting as an administrative assistant is desired -Previous experience in home care, home health, geriatrics or long-term care is desired
