Transition Specialist

Overview

Provides community-based transition support and crisis care for mental health clients.

Tasks

-Completes Wellness Plans, Comprehensive assessments and other documentation as required. -Collaborate with outside community referral agencies, providers and Care Coordination services for aftercare, to build referral options for transiting consumers. -Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency and OMH standards. -Complete “Transition Checklist” with all consumers at various stages of treatment, immediate needs and recommend treatment objectives for the consumer’s service. -Participates in daily morning meeting, and inform team in daily meetings and weekly clinical meetings around consumers ready to transition off the ACT team. -Provide 24-hour crisis intervention on-call services on rotating basis.

Requirements

• master’s degree

What You Bring

-Master’s Degree required -At least one of experience working in human services with individuals with serious mental illness and experience in work with families. -Must be outcome oriented in a fast paced environment with deadlines.
