-Plan activities and build links in the community -Support customers with daily living, personal care and medication -Co-ordinate rotas and guide support workers to deliver great care -Keep support plans and risk assessments up to date -Help people build skills and confidence to live more independently

What You Bring

-You’ll work 29.5 hours per week on a two-week set rota, including day shifts, sleep-ins and weekends. -You’ll work from our Kingston House service in Hull. -You need an Enhanced with barring list DBS check that we -Level 2 in Health and Social Care or equivalent experience -Confidence using tech to update plans and complete training -Experience supporting people with learning disabilities -Great communication skills and a person-centred approach -Calm, resilient and able to work on your own initiative
