The Role
Overview
Coordinate care services, manage support workers, ensure plan reviews, liaise with agencies.

Tasks
-Reporting to a Branch Manager on a weekly basis on activity levels and outcomes for service users -Defining special care needs based upon identification of specific clinical problems and to monitor and review as appropriate -Providing written reports of all complaints, accidents, incidents and untoward events in accordance with written policies -Liaising with external organisations in respect of developing a total package of care for a service users -Undertaking “on call” duties as required by the branch to ensure a 24/7 service is provided. This will be on a rota basis 1 week at a time 1 out of every 4 weeks. -Direct line management for a team of Support Workers, including the management of their weekly workload

What You Bring
-SVQ 2 or higher or the willingness of completing SVQ 3 -Care Sector experience -Experience as a Care Co-Ordinator would be preferred
