Care Co-ordinator

Manage support workers, oversee care plans, and coordinate services for vulnerable clients.

Tasks

-Providing written reports of all complaints, accidents, incidents and untoward events in accordance with written policies -Direct line management for a team of Support Workers, including the management of their weekly workload -Defining special care needs based upon identification of specific clinical problems and to monitor and review as appropriate -Liaising with external organisations in respect of developing a total package of care for a service users -Reporting to a Branch Manager on a weekly basis on activity levels and outcomes for service users -Undertaking “on call” duties as required by the branch to ensure a 24/7 service is provided. This will be on a rota basis 1 week at a time 1 out of every 3 weeks.

What You Bring

-Experience as a Care Co-Ordinator would be preferred -SVQ 2 or higher or the willingness of completing SVQ 3 -Care Sector experience
