Overview

Coordinate services, activities, and resources for seniors at two Arizona communities.

Tasks

-Deliver excellent customer service by supporting residents with basic needs and concerns. -Maintain positive and professional relationships internally and externally. -Organize and lead social events, engagement activities, and educational opportunities. -Identify affordable service providers and negotiate discounts when possible. -Offer educational sessions on health, wellness, life skills, and available support services. -Assist residents in accessing services such as transportation, meals, personal support, financial assistance, and more. -Develop and maintain a Resource Directory of state and local agencies. -Assist residents with applications for support programs such as Medicaid, Medicare, and other benefit programs. -Provide support across both communities while following the assigned weekly schedule. -Coordinate programs, services, and activities for residents. -Document and report resident or family concerns as required. -Serve as a liaison to community agencies and service providers, building a strong network of partnerships. -Engage residents daily to identify needs and provide appropriate referrals.

What You Bring

-Strong working knowledge of community resources and supportive services. -Two or more years of experience in social service delivery, ideally within senior or family populations. -Bachelor’s degree in social work or a related field preferred. -Demonstrated ability to advocate, organize, problem-solve, and deliver consistent results.
