Support young people with complex needs through night‑shift care and planning

Tasks
-Creating support plans with your customers and working alongside a small team of other Support Coordinators working with the plans. -Carrying out risk assessments, support planning, goal setting and regular reviews. -Working with our young customers who will be looked after children or children in need, ensuring that Ofsted Supported Housing needs are met in our service. -Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.

What You Bring
-You’ll need an Enhanced DBS check done and we pay for that. -It is preferred that you are able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues. -Experience of coordinating and assessing customer referrals. -The ability to work on your own initiative, remain calm under pressure and have a resilient approach. -Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
