Tasks
-Implementing and maintaining ISPs in accordance with state regulations that align with Cedarhurst of Newburgh policies for personalized care. -Overseeing the delivery of quality care and services to our residents in compliance with the regulations pertaining to care plans and resident assessments. -Developing and implementing nursing service objectives, policies, and procedures in coordination with organizational goals and objectives, in accordance with Federal and State regulations. -Assessing residents and potential residents for appropriate levels of care with the sense of urgency. -Overseeing the pharmacy and community relationship including ensuring medications are delivered timely, eMAR is accurate, and address any concerns in real time. -Providing nursing coverage and assistance when needed. -Managing and coordinating the work of all nursing and care staff, communicating pertinent resident health and safety information. -Responsible for overseeing the entire clinical department including residents’ assistants, certified nursing assistants, medication technicians, etc. -Managing inventory and ordering nursing care supplies as needed. -Ensuring communities remain compliant with CLIA waver. -Providing interim documentation as required to resident file, attending all scheduled interdisciplinary resident care meetings. -Promoting the physical, personal, and emotional wellbeing of each resident and striving to maintain the ultimate goal, which is to return the resident to their maximum level of self-care and independence, when possible. -Directing communication with surveyors during IDPH surveys and assisting with plan of corrections. -Engaging in efforts to increase occupancy rates while prioritizing the wellbeing of our residents to support them in aging comfortably within the community. -Implementing and maintaining infection control policy and procedures. -Ensuring effective communication is being utilized by community staff with the residents’ families concerning residents’ care, encompassing their physical, personal, and emotional wellbeing. -Supervising and monitoring medications for residents who self-administer if applicable. -Interviewing, hiring, training, supervising, evaluating, and retaining a high-performing resident care team. -Holding service plan meetings with residents and families to discuss service plans. -Striving to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.

While performing the duties of this job, the employee is occasionally required to stand/move, communicate, and identify written information.The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to 50 pounds independently; occasionally lift and/or move up to 200 pounds with assistance. -Working knowledge of best person-directed care practices for seniors, including those with dementia preferred. -Understanding all responsibilities associated with clinical roles and demonstrating the ability to effectively carry them out. -Working well with others and taking direction from management, as well as taking initiative with the capability to engage in conflict intervention and resolution for residents, families, employees, and others as needed. -Must remain flexible and can work weekends as Manager on Duty, when necessary. -Graduation from an accredited nursing school and a current state license for LPN or RN is required at all times, with 3+ years of nursing experience preferred. -Cedarhurst of Newburgh considers the health and safety of its residents, family members, and team members as its highest priorities. All offers of employment are conditioned on completing and passing a background and drug test, participating in testing requirements (such as TB), and using designated PPE when required. -Understanding the residents’ Advance Directives and following their wishes. -Knowledgeable of and experienced in conducting preadmission assessments, training, and education to staff, residents, and family members. -Be able to demonstrate proficiency in Internet browsers (e.g., Explorer, Chrome, and Fire Fox), Microsoft Office programs including Outlook, Excel, Word and Publisher, electronic health records (EHR) systems, and any HR related systems if applicable. -CPR and BLS certification preferred.
