About the Role

The Reflections Clinical Coordinator, reporting to the RN Wellness Director, plans, organizes, and manages all clinical operational aspects of the Reflections dementia care program. Responsibilities include implementing the INVEST mission, adhering to Brandywine standards, providing dementia training, and ensuring compliance with federal, state, and internal policies. The position is full‑time, Monday‑Friday 7 am‑3 pm with alternating weekend coverage, and pays $39‑$41 hourly. The role supports Brandywine Living’s mission, values, and vision, upholds resident rights, and maintains privacy, dignity, and safety. It ensures safety protocols, confidentiality, quality assurance, regulatory compliance, and a harassment‑free workplace. Teamwork is emphasized through cooperative communication, timely follow‑up on issues, positive public relations, and adherence to training, attendance, uniform, and hygiene standards. • Coordinates recruitment and hiring process specifically for the Reflections Care Management team members • Completes staff performance appraisals/disciplinary action • Facilitates general orientation for all staff on Reflections Program • Conducts monthly in services on various dementia related topics for all staff • Responsible for environmental rounds to check for safety of exit doors, resident room safety and unit safety, and adherence to brand standards • Provides direct support to care management staff for behavior symptom management/resident care approaches • Coordinates and facilitates the RCP Training program for primary staff on unit; tracks completion and schedules RCP classes • Suggests residents for restorative potential and assures program is consistently carried out by staff to maintain resident's maximum practicable level of function • Cooperates with other resident services for complete evaluation of care • Assists in establishing linkages with and arranging for services from public and private sources for income, health, mental health and social services • Assists the resident in making applications for, and maintaining, income entitlements and public benefits • Coordinates care planning meetings per state regulations • Coordinates, implements, and monitors all resident service plans with the Wellness Director • Cooperates with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained • Assists with admits, transfers, and discharge of residents as required • Reports to the RN to assess and document resident and family response to interventions • Provides written and/or oral status reports of residents to assure necessary follow‑up actions • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician in accordance with established procedures • Assists the physician on rounds and ensures pertinent information is communicated to and from the physician • Assists in resident rehabilitation activities, including ambulation, emotional support, and monitoring physical therapy results • Provides direct physical and psychological care in emergencies or illness, reports observations and changes in condition to the RN • Prepares and administers medications as ordered by the physician following all medication administration policies • Ensures adequate supplies and equipment are on hand to meet nursing needs of residents • Complies with Controlled Substances policy and procedures, maintaining accurate inventories and logs, and investigates discrepancies • Reviews medication administration records for completeness, accuracy, and adherence to stop‑order policies; reports discrepancies to the RN • Disposes of drugs and controlled substances as required in accordance with established procedures • Monitors residents for side effects and adverse reactions to medications; reports to RN and takes appropriate action • Adheres to all infection control policies and procedures • Administers and documents all treatments as ordered and in accordance with policies and procedures • Maintains accurate resident care records reflecting nursing process, care provided, and resident response; reports all changes to the RN • Assists in the development of resident care assignment sheets • Reviews all resident care manager assignments for completion • Works in partnership with the INVEST Coordinator to ensure a therapeutic and socially engaging environment • Ensures safety of unit through environmental rounds

What You Bring

Candidates must hold an unencumbered LPN license from an accredited program; dementia‑care training is preferred. Experience with dementia populations and prior work in hospitals, assisted‑living, or skilled‑nursing settings is required. The position involves intermittent movement, frequent interruptions, and exposure to emotionally stressful situations, with physical demands that may include lifting up to 50 lb and using fine and gross motor skills. • Utilizes Brandywine standards and standards of professional organizations in conducting resident care • Demonstrates knowledge and skills necessary to safely provide care appropriate to age of residents managed on unit
