The RAI Coordinator works closely with the Director of Care to oversee assessment processes, data analysis, and staff education, while serving as a key contact for external health agencies and contributing to program development.

Coordinate timely completion of MDS assessments for all residents.
Analyze and integrate MDS data with clinical tools, providing leadership on process improvement.
Conduct critical analysis of resident‑level and Home Health Zone data.

Candidates must hold a practical nursing registration, have several years of experience in long‑term care, and demonstrate strong analytical, instructional, and communication abilities.

3–5 years experience in long‑term care; MDS experience preferred.
Registered Practical Nurse (RPN) status with the College of Nurses of Ontario.
Knowledge of nursing rehabilitation, occupational therapy and physiotherapy collaboration.
Familiarity with InterRAI tools, MDS/RUG III classification systems.
Strong analytical, assessment, and data‑interpretation skills.
Ability to design and deliver training for adult learners.
Effective written and verbal communication abilities.
