Residential Case Manager

Description

The case manager provides supportive counseling and helps residents achieve both short‑ and long‑term rehabilitation goals, while also delivering crisis intervention as needed. Core duties include maintaining accurate documentation, monitoring facility safety, delivering quality customer service, and exercising sound judgment during emergencies.

• Provide support services to an assigned caseload of residents.
• Implement individual service plans to achieve rehabilitation goals.
• Document case records, progress notes, and service plan summaries accurately.
• Deliver instruction across 11 service categories through modeling and guidance.
• Manage residence crises, intervene to reduce risk, and notify supervisors.
• Order, supervise, and count medication per housing medication policy.
• Offer counseling to residents as needed.
• Lead recreational, counseling, and rehabilitation groups.
• Escort residents to medical, benefit, or other appointments.
• Coordinate and participate in community activities.
• Monitor facility safety via video surveillance and hourly walk‑throughs.
• Record building activities and events in the communication logbook.
• Perform light maintenance and housekeeping tasks.
• Verify visitor identification and maintain the visitor log.
• Conduct monthly fire drills.
• Use de‑escalation techniques to respond to resident situations.
• Report crises and emergencies promptly and document them on required forms.
• Maintain client case records, progress notes, and service plans per policy.
• Conduct apartment inspections and support daily living skills.
• Attend residential staff meetings.
• Remain on‑site until relief staff arrive.
• Follow all personnel and residential policies and procedures.

Requirements

Applicants must hold a Bachelor’s degree in a human‑service discipline and have a minimum of two years of counseling experience in a residential or mental‑health setting, though relevant education may substitute for part of the experience.
