

Provides resources and support to individuals experiencing homelessness, empowering them towards stability.
The Case Manager role’s main function is to assist clients in breaking the cycle of homelessness using a “whatever it takes” approach. Responsibilities include conducting intake and individualized needs assessments, developing Individualized Service Plans (ISPs), providing referrals and linkage to resources, completing program‑specific assessments, conducting 90‑day reviews, maintaining timely documentation, and safeguarding client confidentiality per HIPAA.
We seek candidates with strong analytical and detail‑oriented skills, the ability to collaborate and build positive relationships, clear verbal and written communication, creativity and initiative, critical thinking and problem‑solving abilities, and a commitment to ethical standards and professional boundaries.
Preferred qualifications include at least two years of experience in healthcare, case management, homeless services, or related fields; however, all education levels and experience levels are welcome.
All candidates must verify employment eligibility, obtain CPR/First Aid training if not already certified, complete a tuberculosis test, background screening, and drug test. Driving is an essential function, requiring reliable transportation, a valid driver’s license, proof of insurance, and the ability to qualify for PATH’s insurance coverage.
Applicants must be authorized to work in the United States; we do not sponsor visas. Employees currently receiving services from PATH or who have within the past two years are ineligible. As a California employer, all regular work must be performed within the state, with on‑site or hybrid schedules as operationally required; fully remote positions are not offered.

Our Supportive Housing Services (SHS) provide Intensive Case Management Services (ICMS) in a Permanent Supportive Housing (PSH) setting for people who have experienced homelessness and have chronic health, mental health, or co‑occurring conditions. Services are centralized on‑site to ensure easy access to resources that promote long‑term housing stabilization, quality of life, and community integration.
