In this role, you will provide housing‑focused case management services to homeless families residing in a Tier II emergency family shelter, working quickly to secure permanent housing.
Westhab complies with all applicable OSHA standards, rules, and regulations, ensuring a safe and healthful work environment free of recognized hazards.
- Collaborate with Westchester County Department of Social Services and other providers to help families achieve housing goals.
- Provide comprehensive case management services, including bi‑weekly Individualized Living Plans.
- Develop housing plans with residents to ensure timely transition to permanent housing and appropriate use of subsidies.
- Refer residents to specific housing opportunities.
- Conduct biweekly room inspections.
- Provide crisis intervention as needed.
- Participate in monthly Town Hall meetings.
- Submit and track work order requests.
- Assist residents with completion and filing of housing and rental subsidy applications.
- Help residents secure moving costs from DSS, including broker fees, security deposits, first month’s rent, and furniture allowance.
- Link families with community‑based providers for long‑term social support and services.
- Maintain accurate written records and required documentation per HUD and WCDSS standards.
- Keep demographic and other resident information for statistical reporting through ClientTrack HMIS and other databases.
- Submit accurate, timely reports as requested.

A bachelor’s degree in social work or a related field is preferred, and strong computer, written, and verbal communication skills are required. Preferred experience includes case management, home visiting, homelessness, or child welfare, along with a strength‑based, trauma‑informed approach. Sensitivity to culturally diverse families is essential, and Spanish fluency is highly desirable.
Benefits
The Housing Case Manager is a non‑exempt position in the Coachman Family Center, reporting to the Service Coordinator. It is located in White Plains, NY, with an annual salary ranging from $40,000 to $45,000.
