Care Coordinator/Assistant Director of Resident Services

Description

• Assists in completing the associate performance evaluations in accordance with established policy.*
• Conducts meetings/training as required/requested.*
• Reviews resident charts and properly files old/outdated records and information.
• Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors, and associates.*
• Provides “hands on” care on an as needed basis to meet the needs of the residents.*
• Sets up new admission charts.
• Accepts calls from staff during nights and weekend.*
• Participates in monthly ECC & LNS meeting with Director of Resident Services and Corporate Director of Nursing.*
• Assists in scheduling associates based upon budgeted FTE's and prepares assignments to ensure efficiency.*
• Assists in reviewing new associate applications, checks references, interviews applicants for the best possible hire.*
• Conducts cart audits ensuring accuracy and proper documentation.
• Assists in performing evaluation for residents to determine level of care and services.*
• Assists in monitoring expenditures weekly to ensure compliance with department budgeted monthly amounts.*
• Prepares staff schedules and posts/makes available in a timely fashion..
• Ensures medication orders are accurate on the MORS.*
• Assists in reviewing and investigating all incidents reports and recommends appropriate action/follow up to the Executive Director.*
• Prepares 1823 forms for new residents, and updates / keeps current 1823 forms for existing residents.
• Assists in coaching/counseling associates in a respectful manner.*
• Assists in monitoring the associates’ work and performance.*

• Understands and adheres to the ALF regulations pertaining to assisted living health care laws.*
• Understands and complies with HIPAA regulations.*

• Knows, understands, and follows established policy and procedures in emergency situations.*
• Demonstrates excellent decision-making skills and can understand the impact of those decisions.*
