Intake Nurse

Description

• Assist the Telephonic Triage team located on-site.
• Operate appropriate patient care equipment and manage computerized information systems.
• Conduct intake assessments for the OB clinic and Chronic Disease clinic.
• Support the Care Coordination team in delivering comprehensive care.
• Manage medical records and coordinate scheduling.

Requirements

• Experience in intake assessments is strongly preferred.
• Current BLS/CPR certification.
• Spanish-speaking skills are highly preferred.
• Ability to operate patient care equipment and enter computerized information efficiently.
• Proficiency in medical records management, scheduling coordination, and case management.
