The City seeks a projects‑driven individual to lead, promote and support change initiatives that improve health outcomes for individuals experiencing homelessness across Toronto. This exciting opportunity involves leading projects that create referral pathways between primary, mental health, emergency and community health services and shelter providers.
Reporting to the Project Director, Homelessness Initiatives and Prevention Services, the Project Manager will collaborate with allied health partners to develop cross‑sector referral pathways and deliver sustainable health and housing outcomes. The incumbent will provide leadership and expert guidance on divisional initiatives aimed at removing barriers to health‑care access throughout the health‑care system.
- Build and maintain strategic relationships with corporate, healthcare, government, and senior management stakeholders to close health‑care access gaps.
- Lead development of cross‑sector agreements, medical directives, policies, and protocols improving health‑care access for the homeless.
- Negotiate and implement innovative multi‑stakeholder solutions to expand health‑care services for people experiencing homelessness.
- Represent the division in meetings with diverse internal and external health‑care partners, community groups, and elected officials.
- Advocate system‑wide improvements to health outcomes for homeless individuals across race, gender, status and income.
- Support evaluation and implementation of the Homelessness Health Services Framework by identifying care barriers.
- Design and execute detailed plans for new programs and system changes across the division and partners.
- Prepare project and staff reports, briefing notes, and status updates for senior leadership.
- Anticipate and analyze emerging issues, recommending innovative organizational and program solutions.
- Lead data‑collection strategy and planning tools to develop solutions for health‑care access challenges.
- Facilitate knowledge transfer to empower site‑level leaders in navigating referral pathways.
- Chair advisory groups to plan and develop program features and implementation plans.
- Supervise, motivate and collaborate with project team members to ensure high‑quality performance and continuous learning.

- Hold a post‑secondary degree in health, social sciences, public administration or equivalent experience.
- Demonstrated experience building strategic organizational and intergovernmental partnerships serving diverse, marginalized communities.
- Extensive large‑scale project management experience in health or social services, including planning, logic models, policy development and evaluation.
- Strong background in advisory and consultation processes with councillors, staff, external partners and community members.
- Proven ability to lead, manage and motivate professional teams toward organizational goals.
- Skilled at securing senior‑leadership alignment for new approaches, change initiatives and health‑best practices.
- Expertise in writing complex reports, briefing notes and staff documents for senior leaders.
- Highly developed communication, presentation and interpersonal skills for persuasive engagement at all levels.
- Political acumen and conflict‑resolution abilities to negotiate consensus among differing objectives.
- Capability to lead change‑management initiatives fostering a progressive, effective program‑delivery culture.
- Strategic, analytical and conceptual thinking to navigate complex policy and performance issues.
- Current knowledge of health inequities, social determinants of health, and housing‑homelessness trends.
- Familiarity with collective agreements, City policies, statutes, municipal by‑laws and legislation such as ESA, Human Rights Code and OHSA.

Job ID 55925 is a full‑time, temporary position (approximately 12 months) in the Toronto Shelter & Support Services division, located at Metro Hall, 55 John Street. The role offers a salary range of $103,431.00 to $141,247.00 annually, with an ideal hiring zone between $112,398.00 and $121,863.00, and a Monday‑to‑Friday, 35‑hour work week.
