  • Support customers to build independence and achieve their goals under the guidance of your Senior Client Service Manager.
  • Assist customers with varied needs including mental health, housing, and complex behaviours.
  • Work with a team of Support Coordinators to deliver a high-quality service that meets Home Group’s KPIs.
  • Create and update support plans and risk assessments, helping customers with daily tasks like cooking, budgeting, and moving to independent living.
  • Ensure properties are clean, safe, and welcoming, helping reduce voids.

  • Experienced in creating person-centred support plans, risk assessments, and reviews.
  • Able to work independently, stay calm under pressure, and demonstrate resilience.
  • Basic Microsoft Word and Excel skills required.
  • Confident using technology for support plans, training, and collaboration.
  • Skilled in coordinating and assessing customer referrals.
  • Enhanced DBS check required (we cover the cost).
  • Empathetic and passionate about supporting customers in housing or care settings.
