The LPN Charge Nurse is responsible for advanced clinical observations, nursing judgments, and actions, ensuring compliance with federal, state, and local regulations, and interpreting policies for staff, families, and physicians. The role includes performing daily rounds, developing care plans, managing medication administration and accounting, supervising nursing staff, monitoring nutrition, coordinating admissions, and maintaining accurate resident documentation.
- Conduct advanced observations, record findings, form nursing judgments, and take appropriate actions.
- Follow established nursing standards and ensure compliance with federal, state, and local regulations.
- Interpret policies for staff, families, and physicians.
- Perform daily rounds, develop comprehensive care plans, and manage medication administration and accounting.
- Assign nursing staff, follow staffing guidelines, and provide accurate resident condition reports.
- Monitor diets, supervise meal trays and snacks, and assist with feeding as needed.
- Coordinate admissions and communicate with departments, staff, physicians, and service providers.
- Ensure staff follow the chain of command according to personnel policies.
- Assign and supervise nursing assistants, document and correct poor performance, and inform management of issues.
- Participate in performance evaluations, providing counseling, coaching, and training.
- Follow written and verbal directions and provide exemplary customer service to residents, families, peers, managers, and visitors.
- Report changes in physical or mental conditions promptly and adhere to all company, facility, and state policies.
- Attend staff meetings, in‑services, interdisciplinary team, and committee meetings as assigned.
- Observe environmental conditions (noise, odors, cleanliness) and cooperate with other departments to maintain quality.
- Document resident information timely, accurately, and confidentially; notify POA and doctors of changes; perform comprehensive assessments.
- Verify and update medication administration records (MARS/TARS) by the first of each month.
- Know resident rights, help residents exercise or protect them, and report complaints while maintaining confidentiality.
- Ensure resident safety through proper use of Wanderguard devices and other safety equipment.
- Demonstrate proper equipment use, report needs or repairs, and follow infection control protocols.
- Practice universal precautions and life safety measures.

The position requires graduation from an accredited nursing or practical nursing program, a current nursing license, and one to two years of experience.
