The Navigator develops and implements a case‑management program that provides psychosocial support, coordinates resident transitions across care levels, and offers resources for grief, depression, illness, loss, and trauma. They ensure cross‑functional departmental support for post‑acute services, collaborate with the wellness and activities teams to create holistic programs, and facilitate communication with residents, families, and key stakeholders during condition changes. Participation in continuity of care, transition, and wellness committee meetings is also required, along with conducting assessments, creating treatment plans, and maintaining records.
Qualified candidates hold an MSW/LCSW, RN license, or a BA/BS in a related field, have at least three years of experience training related to the aging process, and possess case‑management experience on an interdisciplinary team. Experience with Total Quality Management (TQM) is also required.

THE FORUM is recruiting a hospitality‑focused Health Care Navigator to build relationships, coordinate social services, and locate resources for residents transitioning throughout the continuum of care in Independent and Assisted Living. The Navigator evaluates residents’ needs, assists them in accessing resources, and provides educational programs for staff on dealing with cognitive impairments. The goal is to guide residents, families, and caregivers through successful health and wellness transitions to achieve optimal wellbeing and appropriate levels of care.
This is a full‑time exempt position with a salary range of $84,000 to $103,000.
