Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans.
Work in close collaboration with; local authority, NHS mental health teams, safeguarding teams and housing option teams.
Promoting Home Group’s values within your team.
Sporting customers with independent living skills such as; budgeting, maximising customers income, cooking, engaging with external services.

Experience of coordinating and signposting to internal and external services.
Working to a set rota pattern of day shifts, including weekends and Bank Holidays.
Recognise every customer is different and you’re able to adopt a flexible, personal approach, listening to customers to understand their needs.
The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
Able to use technology to update outcome spreadsheets, complete online learning and to collaborate with colleagues or stakeholders.
